Randomised clinical testing for evaluating the integrated management model for chronic disease supported by Information and Communication Technologies (ICT).

Structure of the study: A clinical, open, controlled and randomised test using 3 parallel groups (intervention group with NOMHADchronic, telephone help group and routine care group) developped in the Department of the Valencia Agency for Health in Valencia (multi-center).

The aims of the TECCU project are the following:

– Primary Objective:

  • To evaluate the effectiveness of the NOMHADChronic platform in improving quality of life related to health (CVRS) of those patients with chronic illnesses of high complexity in comparison to the routine care and conventional follow-up usually experienced by those patients.

– Secondary Objectives:

  • To detail mortality rates, visits to the ER and urgent hospital readmissions in the 12 months following the recruitment of patients to the groups NOMHAD, Routine Care and Programmed Follow-Up. Subsequently, to evaluate the efficiency of NOMHADChronic with respect to the reduction of an endpoint combining mortality and/or urgent hospital admissions in the 12 months following the beginning of the medical procedure for those in the Routine Care and Programmed Follow-Up groups.
  • To evaluate the security of NOMHADchronic, judging by the adverse effects that an unprogrammed medical or nursing visit demands.
  • To evaluate the efficiency of NOMHADchronic by analysing the relation of incremental cost-effectivity (the marginal cost of Year of Life Adjusted by Quality (AVAC) – increased in the NOMHAD group in comparison with the Routine Care and Programmed Follow-up groups).
