    Abstract: The study presents some results of customer paths’ analysis in a shopping mall. Bluetooth-based technology is used to collect data. The event log containing spatiotemporal information is analyzed with process mining. Process mining is a technique that enables one to see the whole process contrary to data-centric methods. The use of process mining can provide a readily-understandable view of the customer paths. We installed iBeacon devices, a Bluetooth-based positioning system, in the shopping mall. During December 2017 and January and February 2018, close to 8000 customer data were captured. We aim to investigate customer behaviors regarding gender by using their paths. We can determine the gender of customers if they go to the men’s bathroom or women’s bathroom. Since the study has a comprehensive scope, we focused on male and female customers’ behaviors. This study shows that male and female customers have different behaviors. Their duration and paths, in general, are not similar. In addition, the study shows that the process mining technique is a viable way to analyze customer behavior using Bluetooth-based technology.

    Abstract: Medical emergencies are one of the most critical processes that occurs in a hospital. The creation of adequate and timely triage protocols, can make the difference between the life and death of the patient. One of the most critical emergency care protocols is the stroke case. This disease demands an accurate and quick diagnosis for ensuring an immediate treatment in order to limit or even, avoid, the undesired cognitive decline. The aim of this paper is perform an analysis of how Process Mining techniques can support health professionals in the interactive analysis of emergency processes considering critical timing of Stroke, using a Question Driven methodology. To demonstrate the possibilities of Process Mining in the characterization of the emergency process, we have used a real log with 9046 emergency episodes from 2145 stroke patients that occurred from January of 2010 to June of 2017. Our results demonstrate how Process Mining technology can highlight the differences of the stroke patient flow in emergency, supporting professionals in the better understanding and improvement of quality of care.

    Abstract: The wide-spread adoption of real-time location system is boosting the development of software applications to track persons and assets on real-time and perform analytics. Among the vast amount of data analysis techniques, process mining allows to conform work-flows with heterogeneous multivariate data, enhancing the model understandability and usefulness in clinical environments. However, such applications still find entrance barriers in the clinical context. In this paper we have identified the preferred features of a process mining based dashboard deployed in the operating rooms of a hospital equipped with a real-time location system. Work-flows are inferred and enhanced using process discovery on location data of patients undergoing an intervention, drawing nodes (states in the process) and transitions across the entire process. Analytic Hierarchy Process has been applied to quantify the prioritization of the features contained in the process mining dashboard (filtering data, enhancement, node selection, statistics, etc..), distinguishing on the priorities that each of the different roles in the operating room service assigned to each feature. The staff in the operating rooms (N=10) was classified into three groups: Technical, Clinical and Managerial staff according to their responsibilities. Results show different weights for the features in the process mining dashboard for each group, suggesting that a flexible process mining dashboard is needed to boost its potential in the management of clinical interventions in operating rooms.

    Abstract: Thermal tolerance and the respiratory properties of isolated red muscle mitochondria were investigated in Oreochromis alcalicus grahami from the alkaline hot-springs, Lake Magadi, Kenya. Populations of O. a. grahami were resident in pools at 42.8 °C and migrated into water reaching temperatures of 44.8 °C for short periods. The maximum respiration rates of mitochondria with pyruvate as substrate were 217 and 284 natom O mg-1 mitochondrial protein min-1 at 37 °C and 42 °C, respectively (Q10=1.71). Fatty acyl carnitines (chain lengths C8, C12 and C16), malate and glutamate were oxidised at 70­80 % of the rate for pyruvate. In order to assess evolutionary temperature adaptation of maximum mitochondrial oxidative capacities, the rates of pyruvate and palmitoyl carnitine utilisation in red muscle mitochondria were measured from species living at other temperatures: Notothenia coriiceps from Antarctica (-1.5 to +1 °C); summer-caught Myoxocephalus scorpius from the North Sea (10­15 °C); and Oreochromis andersoni from African lakes and rivers (22­30 °C). State 3 respiration rates had Q10 values in the range 1.8­2.7. At the lower lethal temperature of O. andersoni (12.5 °C), isolated mitochondria utilised pyruvate at a similar rate to mitochondria from N. coriiceps at 2.5 °C (30 natom O mg-1 mitochondrial protein min-1). Rates of pyruvate oxidation by mitochondria from M. scorpius and N. coriiceps were similar and were higher at a given temperature than for O. andersoni. At their normal body temperature (-1.2 °C), mitochondria from the Antarctic fish oxidised pyruvate at 5.5 % and palmitoyl-dl-carnitine at 8.8 % of the rates of mitochondria from the hot-spring species at 42 °C. The results indicate only modest evolutionary adjustments in the maximal rates of mitochondrial respiration in fish living at different temperatures.

Objective: To describe the development, as part of the European Union MOSAIC (Models and Simulation Techniques for Discovering Diabetes Influence Factors) project, of a dashboard-based system for the management of type 2 diabetes and assess its impact on clinical practice. Methods: The MOSAIC dashboard system is based on predictive modeling, longitudinal data analytics, and the reuse and integration of data from hospitals and public health repositories. Data are merged into an i2b2 data warehouse, which feeds a set of advanced temporal analytic models, including temporal abstractions, care-flow mining, drug exposure pattern detection, and risk-prediction models for type 2 diabetes complications. The dashboard has 2 components, designed for (1) clinical decision support during follow-up consultations and (2) outcome assessment on populations of interest. To assess the impact of the clinical decision support component, a pre-post study was conducted considering visit duration, number of screening examinations, and lifestyle interventions. A pilot sample of 700 Italian patients was investigated. Judgments on the outcome assessment component were obtained via focus groups with clinicians and health care managers. Results: The use of the decision support component in clinical activities produced a reduction in visit duration (P«.01) and an increase in the number of screening exams for complications (P textbackslashtextless .01). We also observed a relevant, although nonstatistically significant, increase in the proportion of patients receiving lifestyle interventions (from 69% to 77. Regarding the outcome assessment component, focus groups highlighted the system’s capability of identifying and understanding the characteristics of patient subgroups treated at the center. Conclusion: Our study demonstrates that decision support tools based on the integration of multiple-source data and visual and predictive analytics do improve the management of a chronic disease such as type 2 diabetes by enacting a successful implementation of the learning health care system cycle.

    Abstract: Background Expressing anthropometric parameters (height, weight, BMI) as z-score is a key principle in the clinical assessment of children and adolescents. The Centre for Disease Control and Prevention (CDC) growth charts and the CDC-LMS method for z-score calculation are widely used to assess growth and nutritional status, though they can be imprecise in some percentiles. Objective To improve the accuracy of z-score calculation by revising the statistical method using the original data used to develop current z-score calculators. Design A Gaussian Process Regressions (GPR) was designed and internally validated. Z-scores for weight-for-age (WFA), height-for-age (HFA) and BMI-for-age (BMIFA) were compared with WHO and CDC-LMS methods in 1) standard z-score cut-off points, 2) simulated population of 3000 children and 3) real observations 212 children aged 2 to 18 yo. Results GPR yielded more accurate calculation of z-scores for standard cut-off points (ptextbackslashtextlesstextbackslashtextless0.001) with respect to CDC-LMS and WHO approaches. WFA, HFA and BMIFA z-score calculations based on the 3 different methods using simulated and real patients, showed a large variation irrespective of gender and age. Z-scores around 0 +/- 1 showed larger variation than the values above and below +/- 2. Conclusion The revised z-score calculation method was more accurate than CDC-LMS and WHO methods for standard cut-off points. On simulated and real data, GPR based calculation provides more accurate z-score determinations, and thus, a better classification of patients below and above cut-off points. Statisticians and clinicians should consider the potential benefits of updating their calculation method for an accurate z-score determination.

    Abstract: The following communication describes the interactive development and implementation of a web-based clinical decision support system to help doctors on the assessment of pancreas and kidney transplant in patients with type 1 Diabetes Mellitus. An exploratory data analysis has been performed using a group of 82 patients, which have been transplanted in University Hospital La Fe (Valencia, Spain) between 2002 and 2015. This analysis allows to describe the sample population and 134 variables acquired during the study. Among them, the most significant indicators were used for developing a classification model to predict the transplant success and failure. An ensemble model of k-Nearest Neighbors enables to predict the pancreas loss with a 82,9% accuracy using ten input variables. The model was implemented in runtime engine and integrated into a distributed service oriented architecture.

    Abstract: Stress and emotions can cause several physiological and behavioral changes, some of which may also have a severe impact on the cardiovascular function. For this reason, an accurate assessment of stress level and emotion kind and strength should be considered important to reduce the associated cardiovascular risks. However, the use of biological parameters to reliably evaluate stress and emotions requires appropriate experimental choices and analysis methods to minimize the variability of the measures and ensure a better comparison between different subjects. The aim of the present paper is to provide a critical, though brief, overview of some of the most used biological parameters that can be considered to quantitatively assess stress and emotions.

    Abstract: WHO (World Health Organization) classifies overweight and obesity based on BMI (Body Mass Index). BMI is considered a rough guide because it fails to discriminate between body fat percent and lean mass in both sexes. Obesity is considered a chronic disease associated comorbidities and risk factors. This review considers a variety of medical conditions and risk factors and their association with overweight and obesity, including type of evidence, their presence in Health Electronic Records of Health Department La Fe in Valencia, and their potential to be included in a novel study. This study will use comorbidities and risk factors associated to obesity and Process Mining techniques, to predict obesity in population, without BMI measure, as part of systematic detection model.

Today’s rich digital information environment is characterized by the multitude of data sources providing information that has not yet reached its full potential in eHealth. The aim of the presented approach, namely CrowdHEALTH, is to introduce a new paradigm of Holistic Health Records (HHRs) that include all health determinants. HHRs are transformed into HHRs clusters capturing the clinical, social and human context of population segments and as a result collective knowledge for different factors. The proposed approach also seamlessly integrates big data technologies across the complete data path, providing of Data as a Service (DaaS) to the health ecosystem stakeholders, as well as to policy makers towards a ‘health in all policies’ approach. Cross-domain co-creation of policies is feasible through a rich toolkit, being provided on top of the DaaS, incorporating mechanisms for causal and risk analysis, and for the compilation of predictions.

    Abstract: © 2016 IEEE. Digital Health Literacy (DHL) is a key element to promote patient empowerment. This position paper presents the lessons learnt from the daily activities of a General Practitioner interacting with patients. General Practitioners have a main role in each stage on individual digital health literacy process. They are the first meeting point between patients and the medical knowledge; in the search phase, they are who can prescribe and validate health information; in the comprehension phase, they lead to a full understanding; and in the adoption phase, they assist in the own personal application. Major conclusions are that General Practitioners need a set of tools, organizational resources and knowledge to acquire Digital Health Literacy skills to help patients on their way from the information to the empowerment. Some of these tools and knowledge are identified to draw the future roadmap to get people with Digital Health Literacy skills.

    Abstract: The application of statistics and mathematics over large amounts of data is providing healthcare systems with new tools for screening and managing multiple diseases. Nonetheless, these tools have many technical and clinical limitations as they are based on datasets with concrete characteristics. This proposition paper describes a novel architecture focused on providing a validation framework for discrimination and prediction models in the screening of Type 2 diabetes. For that, the architecture has been designed to gather different data sources under a common data structure and, furthermore, to be controlled by a centralized component (Orchestrator) in charge of directing the interaction flows among data sources, models and graphical user interfaces. This innovative approach aims to overcome the data-dependency of the models by providing a validation framework for the models as they are used within clinical settings.

    Abstract: In this work, we present a framework implemented within the EU project MOSAIC, funded under the FP7 framework, to gather Type 2 Diabetes (T2D) patients’ data coming from three European hospitals and a local health care agency. A subset of the MOSAIC activities is centered on the development of Temporal Data Mining models to identify relevant clinical pathways in patients’ histories and will in particular benefit from the data coming from the medical centers involved in the project. To best exploit this repository, the need for creating a common and sharable data model becomes immediately apparent. This model is the main subject of this paper. The proposed approach relies on the Informatics for Integrating Biology and the Bedside (i2b2) and the Shared Health Research Information Network (SHRINE) open source software tools. It provides an integrated research setting to merge clinical and environmental data that will enable obtaining a broader vision of individual patients’ histories, which will be then mined with multivariate models to identify relevant clinical pathways. © 2014 IEEE.

    Abstract: Current trends in health management improvement demand the standardization of care protocols to achieve better quality and efficiency. The use of Clinical Pathways is an emerging solution for that problem. However, current Clinical Pathways are big manuals written in natural language and highly affected by human subjectivity. These problems make their deployment and dissemination extremely difficult in real practice environments. Furthermore, the intrinsic difficulties for the design of formal Clinical Pathways requires new specific design tools to help making them relly useful and cost-effective. Process Mining techniques can help to automatically infer processes definition from execution samples and, thus, support the automatization of the standardization and continuous control of healthcare processes. This way, they can become a relevant helping tool for clinical experts and healthcare systems for reducing variability in clinical practice and better understand the performance of the system.

    Abstract: We present an approach for re-ranking health videos obtained from YouTube, called Domain-based ranking. Our system automatically identifies videos coming from trusted sources (channels), such as hospitals and health organizations, and re-ranks YouTube results so that such videos are presented first in the ranking list. Video and channel metadata are used to automatically determine if a video is provided by a trusted source. The approach is tested and results show that the amount of relevant and reliable videos ranked within top-10 increase when using Domain-based ranking, compared with the original YouTube ranking.

    Abstract: Semantic technologies are experimenting an increasing popularity in the context of different domains and applications. The understanding of any class of system can be significantly changed under the assumption any system is part of a global ecosystem known as Semantic Web. The Semantic Web would be an evolving extension of current Web model (normally referred as Syntactic Web) that introduces a semantic layer in which semantics, or meaning of information, are formally defined. So, semantics should integrate web-centric standard information infrastructures improving several aspects of interaction among heterogeneous systems. This is because common interoperability models are progressively becoming obsolete if compared with the intrinsic complexity and always more distributed focus that feature modern systems. For example, the basic interoperability model, that assumes the interchange of messages among systems without any interpretation, is simple but effective only in the context of close environments. Also more advanced models, such as the functional interoperability model that integrates basic interoperability model with the ability of intepretating data context under the assumption of a shared schema for data fields accessing, appears not able to provide a full sustainable technologic support for open systems. The Semantic Interoperability model would improve common interoperability models introducing the interpretation of means of data. Semantic interoperability is a concretely applicable interaction model under the assumption of adopting rich data models (commonly called Ontology) composed of concepts within a domain and the relationships among those concepts. In practice, semantic technologies are partially inverting the common view at actor intelligence: intelligence is not implemented (only) by actors but it is implicitly resident in the knowledge model. In other words, schemas contain information and the “code” to interpretate it.

    Abstract: Respiration exercises are an important part in the pulmonary rehabilitation of COPD (chronic obstructive pulmonary disease) patients. Furthermore, previous research has demonstrated that showing respiration pattern helps the patients to improve their breathing skills. We have developed a low cost and non-invasive prototype based on the Wii remote game controller infrared camera to provide BPM (breaths per minute) measurement as feedback. It can also be a comfortable solution without wires, batteries or any kind of electronics but just wearing passive markers. The lab evaluation with 7 healthy individuals showed that this approach is feasible when users are resting of their exercise. The BPM monitored during the tests presented less than 15% of maximum error and the RMSE (root mean square error) was lower than 6% in all the tests. Further research is needed to evaluate and adapt the system for COPD patients. In addition, more work is needed to develop applications that can be built to motivate and guide the users.

    Abstract: Diabetes management has no patterns in the actual health industry. Many models have been presented for monitoring its behavior focusing only in blood glucose values and basing patient evolution on diary records. The presented system describes the architecture for fully monitoring diabetes patients through a platform of subsystems built in different technologies. Internet stands as a solution to overcome interoperability troubles when trying to communicate native different environments. Patient applications sharing services in a dynamic way provides a new way to enhance Personalized Health Systems.

    Abstract: Chronic cardiovascular diseases directly account for millions of deaths, billions of Euros and a big number of disabilities affecting the world’s population. Even though primary and secondary prevention factors are well known, the awareness and the concern of citizens and patients is not big enough to cause a significant change in lifestyle that modifies the increasing trends. Patients and families, professionals and healthcare systems are not prepared to fight against this burden in an effective and aligned way. Some disease management programmes based on ICT solutions have and are currently being tested around the world but their relative impaction has been very limited. This paper proposes a new turn into Personal Health Systems applied to chronic disease management by increasing the capabilities for personalization, providing the patients with motivation and coaching support and enabling the work of the professionals with intelligent tools for strategic and clinical decision making based on the newest medical evidence.

    Abstract: Current trends in health management improvement demand the standardization of care protocols to achieve better quality and efficiency. The use of Clinical Pathways is an emerging solution for that problem. However, current Clinical Pathways are big manuals written in natural language and highly affected by human subjectivity. These problems make the deployment and dissemination of them extremely difficult in real practice environments. In this work, a complete computer based architecture to help the representation and execution of Clinical Pathways is suggested. Furthermore, the difficulties inherent to the design of formal Clinical Pathways in this way requires new specific design tools to help making the system useful. Process Mining techniques can help to automatically infer processes definition from execution samples. Yet, the classical Process Mining paradigm is not totally compatible with the Clinical Pathways paradigm. In this paper, a pattern recognition algorithm based in an evolution of the Process Mining classical paradigm is presented and evaluated as a solution to this situation. The proposed algorithm is able to infer Clinical Pathways from execution logs to support the design of Clinical Pathways.

    Abstract: This paper introduces our preliminary results in the modeling of Life Assistance Protocols, a new vision of medical guidelines and protocols through the lenses of p-Health. In this context the patient’s role in the process is emphasized, the actions to be performed less defined and not only clinical situations considered, but also healthier lifestyle promotion processes accounted for, where the person’s preferences and motivations play a key role. We propose a complete framework, balancing on classical clinical guideline models and covering both the theoretical and the practical aspects of the problem, describing it from conceptualization to the execution environment.

    Abstract: Nowadays, there are a very large number of patients that need specific health support at home. The deployment of broadband communication networks is making feasible the provision of home care services with a proper quality of service. This paper presents a telehomecare multimedia platform that runs over integrated services digital network and internet protocol using videoconferencing standards H.320 and H.323, and standard TV set for patient interaction. This platform allows online remote monitoring: ECG, heart sound, blood pressure. Usability, affordability, and interoperability were considered for the design and development of its hardware and software components. A first evaluation of technical and usability aspects were carried forward with 52 patients of a private clinic and 10 students in the University. Results show a high rate (mean = 4.33, standard deviation–SD = 1.63 in a five-points Likert scale) in the global perception of users on the quality of images, voice, and feeling of virtual presence.

    Abstract: Home telecare services based on broadband communication were established in five locations in Europe. Two different types of telecare unit were developed: one based on a PC or set-top box containing a videoconferencing codec and another on off-the-shelf videoconferencing units. The participants in the project were 13 medical staff, 135 patients and 88 people informally caring for the patients. Questionnaires were used to evaluate user satisfaction with eight telecare services. Almost all participants rated the usability of the system as good or excellent. A total of 105 telecare sessions were scored by the medical staff. Overall, the quality of audio and video communication was judged satisfactory. For the patients and carers, the perceived quality of communication was also satisfactory and did not vary significantly between sites. The medical staff were reasonably satisfied with how the service supported them in their work. Except for the item about being able to support patients in a critical situation, medical staff agreed that an improved quality of health services was offered through telecare. All participants agreed that personal information was treated confidentially and that there was little risk in using the telecare services. The medical staff trusted the assessments they could make remotely while using the telecare system. Although the findings cannot be generalized due to the small number of telecare sessions and the relatively short duration of the experiment, the results encourage further research.

    Abstract: The availability of health care attention at the point of need is one of the key benefits of telemedicine. Home environment and working place are the two scenarios selected in this article to evaluate the impact of a televisit and teleconsultation service. 31 users from four different medical and patient groups participated in this study supported by European Commission ATTRACT project. The experiences, carried out in Madrid and Valencia Spanish sites, benefited from interactive broadband access networks to provide cost-effective telecare services. Key areas analyzed encompass systems usability, clinical outcomes, patients quality of care and infoethic issues. Services advantages were verified and compared both from patients and medical staff points of view. Main benefits pointed out refer to displacement reduction, better communication doctor/patient, provision of comfort or friendliness, more precise therapy follow-up and increases in patients sense of well being.
